Job Description

An exciting opportunity is available for an experienced and compassionate physical therapist specializing in home health care. This contract role focuses on delivering high-quality physical therapy services directly to patients in their residences, supporting improved mobility, independence, and safety.
Schedule:
Monday Friday, 8:00 AM 5:00 PM Full-time 13-week contract duration Your responsibilities will include: Conducting comprehensive physical therapy evaluations, including OASIS assessments. Developing and implementing patient-centered treatment plans aimed at increasing functional outcomes. Providing skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention. Monitoring patient progress, updating care plans, and communicating important changes to physicians and care team members. Educating patients and caregivers on exercise routines, use of assistive devices, home safety, and strategies for optimizing function. Documenting clinical encounters accurately and on time using Home Care Home Base (HCHB). Collaborating with other healthcare professionals, including nurses, occupational therapists, and home health aides. Participating in interdisciplinary meetings and ongoing professional development.
Qualifications:
Active and unencumbered Physical Therapist (PT) license in Florida. At least 1 year of recent home health physical therapy experience; field experience is preferred. Strong working knowledge of OASIS and proficiency with electronic documentation systems (HCHB preferred). Excellent assessment, clinical decision-making, and organizational skills. Reliable transportation, a valid driver's license, and current auto insurance. Strong communication skills and the ability to work independently in a field-based role.
