The Physical Therapist provides patient-centered care within a home health setting, managing approximately 25-35 patients across multiple counties. Responsibilities include developing and coordinating individualized treatment plans, supervising therapy staff, and ensuring positive patient outcomes through comprehensive rehabilitative interventions.
Key Duties:
Assess, plan, and implement age-appropriate therapeutic care following physician orders. Coordinate multidisciplinary services throughout the patient's episode of care, adjusting plans as needed. Supervise PTAs and Home Health Aides, ensuring quality care delivery. Educate patients and caregivers to promote independence and self-care. Communicate effectively with team members, physicians, patients, and families regarding patient progress and changes. Ensure documentation compliance with regulatory guidelines and participate in performance improvement activities. Adhere to safety, legal, and regulatory standards, including CMS, State, and Joint Commission requirements.
Qualifications & Skills:
Graduation from an accredited physical therapy program. Current licensure as a Physical Therapist and BLS certification. Minimum of 1 year of full-time PT experience, preferably in home health. Strong assessment, rehabilitation, communication, and organizational skills. Proficiency with home health software, internet, and Microsoft Office tools. Valid driver's license and private vehicle insurance.
