Job Description

Deliver meaningful care by supporting patients as they regain strength and independence in their own homes. This contract opportunity as a Home Health Physical Therapist Assistant (PTA) offers a flexible, field-based role assisting individuals to achieve improved mobility and quality of life. Work under the supervision of a licensed Physical Therapist, implementing individualized treatment plans while serving as an essential connection between patients and their healthcare goals.
Schedule & Job Details:
13-week contract, full-time Monday Friday, 8:00 AM 5:00 PM Manage an average caseload of 2532 points per week (typically 57 home visits per day) Daily travel to patient homes within assigned area (mileage reimbursement included) Utilize Home Care Home Base (HCHB) for electronic documentation
Qualifications:
Active Physical Therapist Assistant (PTA) license (state-specific, in good standing) Minimum one year of recent Home Health PTA experience; field experience preferred Experience with Home Care Home Base (HCHB) and understanding of OASIS documentation preferred Sound clinical assessment, observational, and communication skills Reliable transportation, valid drivers license, and auto insurance required Excellent time management, organizational, and independent work abilities
Key Responsibilities:
Deliver prescribed therapeutic treatments to improve patient mobility and function Assist with gait training, transfer techniques, balance, and exercises Monitor and document patient progress, communicating updates to the supervising PT Educate patients and caregivers on safety, exercise, assistive device use, and strategies for home-based success Engage collaboratively with an interdisciplinary healthcare team Maintain up-to-date and accurate case notes within HCHB Participate in care conferences and adhere to all agency, regulatory, and Medicare requirements Enjoy weekly pay, comprehensive medical, dental, and vision insurance options, 401(k) retirement plans, and 24/7 support. Qualified candidates are encouraged to apply today to begin making a difference in home health. $1,542 - $1,631/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates.
