Work closely with patients in their homes, supporting their mobility, strength, and independence while collaborating with a licensed Physical Therapist. This contract position offers a structured weekday schedule with the autonomy and variety that comes with field-based work.

Schedule and Details:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Approximately 2532 points per week (57 patient visits per day) Daily travel within an assigned territory (mileage reimbursement provided) Utilize electronic documentation via Home Care Home Base (HCHB)

Required Qualifications:

Active Physical Therapist Assistant (PTA) license in good standing At least 1 year recent Home Health PTA experience (field experience preferred) Proficiency with Home Care Home Base (HCHB) preferred Familiarity with OASIS documentation processes preferred Reliable transportation, valid drivers license, and auto insurance Ability to work independently, demonstrate strong clinical observation, and communicate effectively Excellent organization and time management skills

Core Responsibilities:
