Job Description

Looking for an opportunity to make a difference in patients lives through individualized, field-based care? A contract opening is available for a skilled Physical Therapist Assistant (PTA) to support home health patients under the instruction of a licensed Physical Therapist. This role allows you to provide impactful, hands-on therapy in the comfort and security of patients homes, emphasizing one-on-one care and support throughout the recovery process.
Schedule:
Full-time, 13-week contract Monday Friday, 8:00 AM 5:00 PM Approximately 2532 points per week (typically 57 patient visits per day)
Qualifications:
Active and unrestricted State PTA license Minimum of one year of recent field-based Home Health PTA experience preferred Experience with electronic documentation systems, especially Home Care Home Base (HCHB), is a plus Familiarity with OASIS documentation and Medicare guidelines preferred Strong communication, clinical observation, and interpersonal skills Reliable transportation, valid drivers license, and current auto insurance Self-motivated, well-organized, and able to thrive independently
Key Responsibilities:
Deliver therapeutic interventions including exercises, gait, balance, transfer, and functional mobility training as per plan of care Track and document patient progress and update the supervising PT regarding changes or improvements Educate patients and caregivers on home exercise programs, safety, and strategies for continued rehabilitation Ensure timely and accurate documentation in HCHB Collaborate with interdisciplinary healthcare teams and participate in required meetings Adhere to agency policies and home health regulations Benefits include weekly pay, a range of health insurance choices, 401(k) options, comprehensive 24/7 support, mileage reimbursement, and the potential for contract extension.
