Job Description
Wellstar Health System is seeking a compassionate Physical Therapist to provide PRN, patient-centered rehabilitative care across our Georgia locations. In this role, you will assess, plan, and deliver evidence-based physical therapy treatments to restore function, reduce pain, and improve mobility for diverse patient populations. Collaborate with physicians, nurses, and interdisciplinary team members to develop individualized care plans, document progress, and ensure safe discharges. Responsibilities Evaluate patients' physical function, mobility, strength, and pain levels using standardized assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercises, manual therapy, and modalities to restore function and reduce pain Educate patients and families on home exercise programs, safety, and injury prevention Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care Document evaluations, treatments, and progress notes accurately in the EMRAdjust treatment plans based on patient response and clinical outcomes Ensure compliance with regulatory, ethical, and safety standards Participate in quality improvement and patient safety initiatives Provide flexible PRN coverage across assigned units or facilities as needed Required Skills Physical therapy evaluation and treatment planning Orthopedic and neurologic rehabilitation techniques Manual therapy and therapeutic exercise Gait training and balance retraining Use of rehabilitation modalities (e.g., heat, cold, electrical stimulation) Electronic medical record (EMR) documentation Patient and family education Interdisciplinary care coordination Clinical reasoning and outcome measurement Infection control and patient safety practices
