Job Description

Role Overview:
The physical therapist assesses, plans, and implements rehabilitative programs to improve mobility, reduce pain, and prevent deformities in patients with disease or injury. Services are delivered across the continuum in a cost-effective, quality-focused, and patient-centered manner, ensuring compliance with regulatory standards.
Key Responsibilities:
Treatment Planning & Care:
Implements treatment plans, selects appropriate activities, revises goals with patient/family input, provides education, discharge instructions, and referrals.
Patient Assessment:
Establishes and evaluates treatment programs, collaborates with physicians, and develops discharge plans aligned with patient goals.
Documentation:
Records evaluations, treatment sessions, and discharge information per departmental guidelines.
Professional Conduct:
Maintains ethical standards, confidentiality, punctuality, effective communication, and teamwork.
Customer Service & Professional Development:
Demonstrates excellent service, applies AIDET principles, fosters positive environment, and participates in ongoing education and training.
Qualifications:
Bachelor's, Master's, or Doctorate in Physical Therapy Licensed Physical Therapist and BLS certification Effective communication, customer service orientation, team collaboration, and basic computer skills This role emphasizes compassionate patient care, adherence to standards, teamwork, and continuous professional growth to support improved patient outcomes and service excellence.
