Job Description

Role Overview:
This position involves evaluating and treating patients primarily in rehab and LTAC hospitals, with responsibilities including conducting initial and discharge assessments, developing and implementing treatment plans, and documenting progress in the EMR. The role requires effective communication with patients, families, staff, physicians, and vendors, as well as participating in interdisciplinary team activities.
Key Responsibilities:
Perform initial and discharge evaluations, set treatment goals, and document appropriately. Develop and deliver timely, goal-oriented treatment plans. Assess educational needs and provide PRN therapy as needed. Collaborate as a member of the healthcare team. Serve as a mentor or clinical instructor for interns when required. Participate in hospital and discipline-specific skills assessments.
Qualifications & Skills:
Bachelor's Degree in Physical Therapy required; Doctoral Degree preferred. GA Physical Therapy license or eligibility, along with BLS certification from the American Heart Association. Minimum of 1 year experience in rehab, SNF, LTACH, or acute settings preferred. Basic computer literacy.
