Summary We are seeking a Part Time Physical Therapist to join our team at Kokua Healthcare. In this role, you will provide high-quality physical therapy services to clients in their homes, helping them achieve their rehabilitation goals and improve their overall well-being. Responsibilities Conduct thorough assessments of patients' physical abilities and limitations. Develop personalized treatment plans based on individual needs and goals. Implement therapeutic interventions to enhance mobility and function. Educate patients and families on treatment plans and home exercises. Monitor patient progress and adjust treatment plans as necessary. Collaborate with other healthcare professionals to ensure comprehensive care. Maintain accurate documentation of patient care and progress. Requirements Valid state license to practice as a Physical Therapist. Strong knowledge of medical terminology and physical therapy techniques. Experience in home health or community care settings preferred. Ability to work independently and manage time effectively. Excellent communication skills for interacting with patients and families.
