PRN, part-time and full-time opportunities available. Who We Are Optimae LifeServices, Inc. provides health care and human services for individuals with disabilities and mental health conditions. We pride ourselves on ensuring those we serve — our customers — are living their life to the fullest with the greatest level of independence. Our rehabilitation services can be provided from the outpatient clinic in Des Moines' East Village, a customer's home or within the community; we meet our customers where they are in their recovery journey. We continue to grow throughout Iowa, now serving Polk, Story, Jasper, Warren and Dallas counties.

Required:
Graduate of physical therapy program approved by the Commission for Accreditation of Physical Therapy Education Licensed to practice physical therapy in Iowa Reliable vehicle and driver's license
