Physical Therapist (PT) - Laurens, IA Blue Stone Therapy is seeking a Physical Therapist to provide rehabilitation services to adult and geriatric patients at Laurens Care Center in Laurens, IA. This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence. $5,000 sign-on bonus available for eligible full-time candidates. What You'll Do - These will be different based on the position
- Provide skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting
- Perform patient evaluations and develop plans of care
- Deliver therapy focused on gait training, balance, mobility, strength, range of motion, endurance, and pain management
- Monitor patient response to treatment and adjust interventions in collaboration with the therapy team
- Maintain appropriate patient schedules per established guidelines
- Complete timely and accurate documentation to support quality care and reimbursement requirements
- Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes Requirements What You Bring -
- Graduate of an accredited Physical Therapy program
- Current PT (or eligibility for licensure) in the applicable state
- Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
- Strong clinical reasoning, communication, and organizational skills
