The position involves managing a caseload of approximately 2532 points per week, typically translating to 57 patient visits daily, depending on the type of care provided. Leveraging tools like Home Care Home Base (HCHB) for electronic documentation and OASIS for patient assessments, you will help clients regain mobility, safety, and independence in a field-based role with daily travel within your assigned territory (mileage reimbursement provided).

Qualifications:

Current, active Physical Therapist license (state-specific, in good standing) At least one year of recent home health physical therapy experience, including fieldwork Strong working knowledge of OASIS assessments and HCHB documentation preferred Competent clinical decision-making and functional assessment skills Reliable transportation, valid drivers license, and auto insurance Independent and well-organized with excellent communication and time management

Key Responsibilities:
