Schedule and Work Details:

Full-time, 13-week contract Monday through Friday, 8:00 AM 5:00 PM Manage a caseload of approximately 2532 points weekly (about 57 patient visits a day, based on visit type) Field-based role with daily travel; mileage reimbursement included Electronic documentation through Home Care Home Base (HCHB) Complete OASIS assessments as appropriate

Key Responsibilities:

Perform comprehensive physical therapy evaluations, including Start of Care and other OASIS-required assessments Build and implement customized treatment plans targeting improved mobility, strength, balance, and safety Deliver therapeutic interventionssuch as exercise, neuromuscular re-education, and gait and transfer trainingwhile adapting strategies as patient needs evolve Educate patients and caregivers on safe mobility, exercises, and the use of adaptive equipment Communicate changes in patient status and progress to healthcare team members and physicians Maintain timely, accurate electronic records Participate in interdisciplinary meetings and keep current with required training

Qualifications:

Active Physical Therapist license (PT) in good standing At least 1 year of recent, relevant Home Health experience Hands-on experience with Home Care Home Base (HCHB) preferred OASIS assessment experience required Reliable transportation, a valid drivers license, and current auto insurance Strong clinical reasoning, organization, and communication skills; ability to self-direct in a field-based environment Benefits include weekly pay, comprehensive health insurance options, 401(k), 24/7 support, mileage reimbursement, and the potential for contract renewal.
