Additional Benefits & Perks Benefits Start Day One Medical, Dental, Vision, and more $5,000 Annual Tuition Reimbursement Support for continuing education and professional development Company Vehicle & Fuel Benefit Program Qualifying employees may receive a company-provided vehicle and a 100% company-funded fuel card As a Home Health Physical Therapist , you will: + Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function. + Assess and document patients' physical strengths to support physician evaluation and guide treatment goals. + Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team. + Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. + Document patient observations, interventions, and evaluations promptly and thoroughly. + Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries. Required Qualifications:
+ Current and unrestricted Physical Therapy license + Current CPR certification + Experience collaborating with a team of healthcare professionals
Preferred Qualifications:
+ One year of experience as a Physical Therapist Pay Range
  • $76.00
  • $106.00 pay per visit/unit
  • $111,600
  • $153,600 per year base pay
Additional Information TB Statement:
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
