This is a PRN position. We are seeking a dedicated Physical Therapist to join our team at Carriage Cove Rehab & Care, located in Rexburg. In this role, you will play a vital part in enhancing the quality of life for our residents by developing and implementing personalized treatment plans that align with our mission of providing exceptional care. This is a PRN position. Responsibilities Evaluate patients' physical abilities and limitations to develop appropriate treatment plans. Implement therapeutic interventions to improve patients' mobility and overall function. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Educate patients and their families on treatment plans and home exercises. Monitor patient progress and adjust treatment plans as necessary. Maintain accurate documentation of patient assessments and treatments in EMR systems. Participate in discharge planning to ensure continuity of care. Stay updated on best practices and advancements in physiotherapy. Doctor of Physical Therapy (DPT) degree from an accredited program. Valid state licensure as a Physical Therapist. Strong knowledge of anatomy, physiology, and medical terminology. Experience with acute care settings preferred. Familiarity with EMR systems is a plus. Excellent communication skills for patient interaction and teamwork. Ability to develop effective care plans tailored to individual needs. Experience in outpatient or home health settings is beneficial.
