Job Description
Olympia Chiropractic and Physical Therapy is seeking a licensed Physical Therapist (PT/DPT) to join our collaborative, multi-disciplinary team. We are a non-surgical, drug-free healthcare organization committed to transforming patient lives by integrating the expertise of Physical Therapists, Chiropractors, PTAs, and Massage Therapists. This position is perfect for a therapist seeking supplementary income, professional flexibility, or a focused clinical environment. We are looking for a commitment of 15-20 hours a week , with a mix of morning afternoon/shifts.
Core Responsibilities:
Focus on the clinical autonomy and collaborative care model:Clinical Evaluation:
Perform comprehensive initial evaluations, develop evidence-based, goal-oriented treatment plans, focusing on functional and work-related outcomes.Integrated Care:
Actively coordinate patient care plans with Chiropractors, PTAs, and Massage Therapists to ensure a holistic, seamless recovery path.Patient Progression:
Implement therapeutic interventions and appropriately progress patients through their care trajectory until functional goals are achieved.Documentation Excellence:
Maintain timely and accurate clinical documentation, including initial evaluations, daily treatment notes, progress summaries, and discharge reports, adhering to all regulatory and internal standards.Discharge Management:
Conduct effective discharge planning and appropriate patient follow-up to ensure long-term success.Requirements & Ideal Candidate Profile Licensure:
Must hold a valid, current, and unrestricted Physical Therapist (PT/DPT) license in the state of Illinois.Experience:
