Requirements:

Minimum of 2+ years of relevant experience

Key Responsibilities:

Collaborate with the care team to develop therapy plans and discharge strategies Perform patient reassessments and update care plans based on responses Document evaluations, treatments, and outcomes accurately in medical records

Work 40 hours per week with 8-hour shifts
