Job Description

Pay:
$80.00 to $120.00 - the pay varies per visit Crystal Home Health Care is looking for an energetic Physical Therapist to service patients in the Chicago Downtown, Chicago South and neighboring cities. Also, the neighboring areas of: South Loop Join our DYNAMIC team! Great People, Great place to work! Physical Therapists are responsible for providing conducting skilled evaluations for our patients and implement a plan of treatment to facilitate increased independence.
Job Responsibilities:
Patient Assessment :
Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.
Treatment Implementation :
Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management and in accordance with the patients care plan, doctor's referral, and individualized treatment plan.
Education :
Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.
Collaboration :
Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs to assure effective coordination and appropriate resource referrals
Documentation :
Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards
Qualifications:
Applicant must be licensed in the state of Illinois Minimum of one year of clinical experience is preferred Minimum of one year of home health experience is preferred Current CPR card Have reliable transportation
