Job Description

PRN Home Care Physical Therapist Salary Range:

$83.20 for Evaluation visits $62.90 for Supervisory/Discharge Visits The Physical Therapist works to improves the level of independence of Home Care patients by: Providing prescribed exercises and therapy per physician orders Review and evaluate physician's referrals Review patients medical records to determine appropriateness Plan and prepare a written treatment program Instruct physical therapist assistant in treatment regime

REQUIREMENTS

Current IL Physical Therapist License IL Driver's License and appropriate reliable transportation required As a public health department, we do have some medical and vaccination recommendations for employees with declinations available.
