Role Overview:

Provides direct patient care activities within a home environment, including assessment, treatment planning, and supervision of physical therapy practices.

Key Responsibilities:

Instructs and assists patients with physical activities and use of supportive devices. Educates patients and families on treatment procedures. Re-evaluates patients and adjusts treatment plans for optimal outcomes. Collaborates with physicians and healthcare practitioners to coordinate care and modify treatment as needed. Evaluates, fits, and adjusts prosthetic and orthotic devices, recommending modifications.

Qualifications:

Bachelor's Degree in Physical Therapy (PT). Valid Driver's License and Physical Therapist License. No prior experience required.

Skills & Attributes:

Excellent communication and instructional skills. Ability to adapt treatment plans based on patient progress. Strong collaboration skills for interdisciplinary coordination.
