Licensed or eligible for licensure as a Physical Therapist in theState of IL Education:
Graduation from a program in physical therapy approved by the Committee on Allied Health Education and Accreditation of The Medical Association or the American Physical Therapy Association or its educational equivalent, or The Council on Medical Education of the American Medical Association and theAmerican Physical Therapy Association Certification:
