Pride Health is hiring a Physical Therapist (PT) to support our client's healthcare facility in Granite City, IL.

Pay Range:

$50.00 to $57.00/hour

Mileage Reimbursement Rate (if applicable): $0.51/ mile

•Pay offered is based on experience, expertise, credentialing, and education.

Duties:

Conduct home health physical therapy admissions and evaluations.

Perform discharge visits and assess patient progress toward established goals.

Develop and implement individualized physical therapy plans of care.

Provide skilled PT interventions in the patient's home.

Monitor patient progress and modify the plan of care as needed.

Supervise and provide clinical direction to Physical Therapist Assistants (PTAs).

Communicate effectively with patients, families, physicians, and the interdisciplinary care team.

Complete required clinical documentation accurately and timely.

Maintain compliance with home health standards, policies, and regulations.

Requirements:

• Current Illinois Physical Therapist (PT) license — required.
• Illinois is not a compact state, so an active Illinois license is required.
