In this role, you will evaluate, develop, and implement individualized treatment plans that help patients restore function, improve mobility, reduce pain, and maximize independence. As a valued member of our multidisciplinary rehabilitation team, you will collaborate with physicians, nursing staff, occupational and speech therapists, case management, and rehabilitation support staff to deliver exceptional, patient-centered care. Whether working with inpatient, outpatient, or long-term care patients, you'll have the opportunity to make a meaningful difference in the lives of those you serve. Essential Responsibilities Evaluate patients and develop individualized treatment plans. Provide skilled physical therapy treatments using evidence-based techniques. Monitor patient progress and modify treatment plans as appropriate. Educate patients and caregivers on home exercise programs, mobility, safety, and use of assistive devices. Collaborate with physicians and interdisciplinary healthcare teams to optimize patient outcomes. Complete accurate and timely clinical documentation. Participate in discharge planning and continuity of care. Mentor Physical Therapist Assistants, students, and rehabilitation support staff as appropriate. Participate in quality improvement initiatives and continuing education. Maintain compliance with HIPAA, accreditation standards, and HMC policies. Qualifications Graduate of an accredited Physical Therapy program (DPT preferred). Current Illinois Physical Therapist license or eligibility for licensure. Current CPR/BLS certification or ability to obtain upon hire. One to two years of clinical experience preferred; new graduates are encouraged to apply. Strong clinical assessment, communication, and critical thinking skills. Ability to work effectively within a collaborative healthcare team.
