Job Description

About Us We are seeking a compassionate, dependable, and clinically skilled Physical Therapist (PT) to provide high-quality skilled nursing care to homebound patients throughout the community. What You'll Do Perform skilled physical therapy evaluations and treatments in the patient's home and develop individualized care plans. Provide skilled physical therapy in accordance with physician orders and the plan of care. Monitor patient progress and communicate changes to physicians and the interdisciplinary team. Perform thorough assessments and document patient progress. Help patients achieve their highest level of independence Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team. Complete accurate and timely documentation in accordance with Medicare and agency requirements. Be willing and qualified to complete OASIS assessments for therapy-only cases, including: Start of Care (SOC) Recertification Resumption of Care (ROC) Discharge Ensure OASIS documentation is accurate, complete, and submitted within required timeframes. Provide patient and caregiver education regarding safety, mobility, exercises, and home programs. Maintain compliance with Medicare, state, and agency policies and procedures. What We're Looking For Required Current, unrestricted Physical Therapist license in IL. Graduate of an accredited Physical Therapy program. Must be willing and able to complete SOC, Recertification, ROC, and Discharge OASIS assessments for therapy-only cases Strong clinical assessment, communication, organization, and documentation skills. Reliable transportation and willingness to travel within the assigned service area. Ability to work independently and manage a flexible schedule. Home health experience preferred. OASIS experience strongly preferred.

$100,000.00 - $115,000.00 per year
