Aim Home Health is seeking an experienced, compassionate a Physical Therapist to provide skilled physical therapy services to patients in their homes. This is a PRN, per-visit paid position. The ideal candidate is self-motivated, organized, and experienced in providing home health care while delivering exceptional patient-centered services. Areas of coverage is DuPage County including Naperville, Lisle, Glendale Heights, Wheaton, Warrenville, and surrounding communities. Responsibilities Evaluate patients and develop individualized physical therapy treatment plans. Provide skilled physical therapy services in accordance with physician orders and the Plan of Care. Assess functional mobility, strength, balance, gait, pain, and safety. Implement therapeutic exercises, gait training, balance training, neuromuscular re-education, and other skilled interventions. Educate patients and caregivers on home exercise programs, fall prevention, safety, and use of assistive devices. Monitor patient progress and modify treatment plans as appropriate. Collaborate with physicians, nurses, and the interdisciplinary team to coordinate patient care. Complete OASIS assessments and maintain accurate, timely documentation in the EMR. Participate in care conferences and discharge planning as needed. Qualifications Current Illinois Physical Therapist (PT) license in good standing. Graduate of an accredited Physical Therapy program. Home health experience is required. Experience with Medicare regulations and OASIS documentation preferred. Excellent clinical assessment, communication, and documentation skills. Valid driver's license, reliable transportation, and current auto insurance.
