Job Description

Powerback Rehabilitation is seeking a Physical Therapist to deliver outcome-driven, patient-centered care across post-acute, outpatient, and home-based settings. In this role, you will evaluate patients, develop individualized treatment plans, and provide evidence-based interventions to restore function and independence. You'll collaborate closely with an interdisciplinary team, educate patients and caregivers, and track measurable outcomes. Responsibilities Provide evidence-based physical therapy evaluations and develop individualized treatment plans for post-acute, outpatient, and home-based patients. Deliver therapeutic exercises, manual therapy, gait training, and functional mobility interventions to restore function and independence. Collaborate with an interdisciplinary team, including physicians, nurses, and other therapists, to coordinate and optimize patient care plans. Educate patients, families, and caregivers on home exercise programs, safety, and strategies to maintain gains and prevent re-injury. Document assessments, goals, progress notes, and outcomes accurately and timely in the electronic medical record. Track and report functional outcomes, using standardized measures to support quality, compliance, and performance improvement. Participate in case conferences, team meetings, and clinical reviews to adjust care plans based on patient progress. Mentor or support therapy assistants and new clinicians, promoting best practices and continuous learning. Contribute to program development and quality initiatives to enhance patient-centered, outcome-driven rehabilitation services. Adhere to all regulatory, ethical, and company policies, ensuring patient safety, privacy, and professional standards of care.
