The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.
Qualifications:
Baccalaureate or master's program in physical therapy approved by an accredited organization Current Illinois license as a Physical Therapist Current CPR card Valid IL driver's license and auto insurance policy Home Care Home Base experience preferred
