Work Environment The Physical Therapist role is primarily onsite, where you will engage directly with patients in their homes. Our operational model emphasizes collaboration among team members while promoting a supportive and dynamic workplace culture. You will work Monday through Friday, engaging with patients in a variety of settings which may include private residences, assisted living facilities, or outpatient clinics. Key Responsibilities As a Physical Therapist at Forest Manor Rehabilitation Services, your daily responsibilities will include: Conducting thorough assessments, gathering pertinent medical history, and performing detailed evaluations to create tailored treatment plans. Collaborating with patients and their families, clearly explaining evaluation findings and incorporating their input into personalized therapy goals. Implementing evidence-based treatment protocols that reflect the unique needs of each patient and adapting these plans based on patient responses and changes in their health status. Maintaining clear communication with interdisciplinary teams, documenting changes in patient conditions requiring adjustments to care plans, and championing necessary referrals for specialized care. Completing discharge planning and follow-up reviews, ensuring timely and accurate documentation of all therapeutic activities, including comprehensive evaluations and progress reports. Demonstrating effective time management to meet productivity metrics while maintaining flexibility to adapt to real-time patient needs and scheduling changes. Engaging in the development of departmental goals, participating in team-building activities, and contributing to community outreach initiatives that enhance access to health care. Embracing ongoing education and training initiatives to enhance your professional competency and to serve as a mentor to clinical students. Promoting exceptional patient experiences by fostering rapport and trust, adjusting approaches to meet the diverse needs of our patient population. Patient Population / Client Focus In this role, you will primarily serve a diverse clientele, including elderly individuals recovering from surgeries, stroke patients regaining mobility, and individuals managing chronic pain conditions. Your interventions will significantly impact their recovery and functionality, as you focus on empowering them to achieve their rehabilitation goals. Qualifications To be considered for this role, candidates must possess: A Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred. State licensure as required for practice in Illinois. Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association. A deep commitment to patient-centered care and the ability to work collaboratively in a team-driven environment. Preferred Experience While prior work experience in physical therapy is not mandatory, candidates with experience in geriatric care, outpatient rehabilitation, or home health settings will find themselves well-prepared for this role. Familiarity with rehabilitation software systems and electronic health records will be advantageous. Compensation and Benefits Forest Manor Rehabilitation Services offers a competitive salary package ranging from $90,000 to $100,000 annually, commensurate with experience.
