Enhanced education programs Discounted CEUs through our Certified Education Division Medbridge Subscription - unlimited CEUs Clinical and Management Career Paths Residency programs available Position Purpose As a member of the therapy team, the Physical Therapist's primary function is to provide a thorough therapy evaluation, diagnosis, a detailed plan of care including treatment methods and procedures; supervision and guidance of Assistants and consultation with referral sources and other healthcare providers. Requirements Qualifications Licensed or eligible for licensure by the State of Illinois under the Illinois Physical Therapy Practice Act. Member or eligible for membership in the APTA preferred. General knowledge and therapy skill to perform the evaluation and treatment of a wide variety of conditions; and ages from neonates, pediatrics, young adults, and adults to geriatrics. Current certification in CPR and other service line specific competencies. Essential Job Functions Perform examinations and evaluations; establish a Physical Therapy diagnosis and treatment plan. Provide evidenced-based Physical Therapy treatment. Documentation, reporting, and information dissemination.
Case management functions:
evaluate effectiveness, revise the plan, and discharge from services. Demonstrate Quality and Service Excellence Demonstrate behaviors that show a positive attitude, empathy and professionalism. Demonstrate interpersonal skills by making customers feel welcome and respected as individuals; protecting their privacy and confidentiality.
