Job Description

Renewal Rehab seeks a compassionate Physical Therapist to provide evidence-based care for seniors in skilled nursing and post-acute care settings. In this on-site role, you will perform thorough evaluations, develop individualized treatment plans, and deliver interventions that improve mobility, safety, and quality of life. You'll collaborate within an interdisciplinary team, document outcomes accurately, and engage in mentorship and professional development, all within a culture that values integrity, respect, open communication, and clinical excellence for older adults. Responsibilities Evaluate senior patients in skilled nursing and post-acute settings using standardized assessments Develop and implement individualized, evidence-based treatment plans focused on functional outcomes and safety Provide therapeutic exercise, balance, gait, and mobility training to maximize independence Collaborate closely with an interdisciplinary care team and communicate progress with families and staff Document evaluations, treatment sessions, and outcomes accurately in the EMR and per regulatory standards Contribute to a patient-centered, respectful culture aligned with Renewal Rehab's values Participate in mentorship, in-services, and professional development opportunities Required Skills Geriatric physical therapy Therapeutic exercise prescription Neuromuscular re-education Balance and gait training Functional mobility and transfer training Post-acute and SNF rehab treatment planning Patient and family education Interdisciplinary care coordination Outcome measure use and documentation EMR documentation
