Overview Join our dynamic healthcare team as a PRN (as-needed) Physical Therapist, where your expertise will directly impact patient recovery and well-being. In this role, you will provide exceptional outpatient and inpatient rehabilitation services, working flexibly to meet the diverse needs of our patients. Your clinical skills and compassionate approach will help patients regain mobility, reduce pain, and improve their quality of life across various settings including hospitals, outpatient clinics, and community care environments. Duties Assess patients' physical conditions and develop personalized treatment plans to promote optimal recovery outcomes. Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology, pediatrics, geriatrics, and community care strategies. Monitor patient progress regularly, adjusting care plans accordingly to ensure effective rehabilitation. Document all patient interactions and treatment activities accurately in electronic health records (EHR) systems following clinical documentation standards. Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate comprehensive patient care. Conduct discharge planning to facilitate smooth transitions from hospital or outpatient settings to home or community environments. Educate patients and their families on treatment procedures, home exercises, and safety measures to support ongoing recovery outside clinical settings. Skills Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy principles essential for effective treatment planning and execution. Proven experience working with geriatric patients, pediatric populations, neurological cases, orthopedics, and outpatient rehabilitation centers. Skilled in developing tailored care plans that address individual patient needs across inpatient hospital units, outpatient clinics, home health visits, and community programs. Familiarity with EMR (Electronic Medical Records) systems and healthcare documentation standards to ensure accurate clinical records management. Excellent client communication skills to motivate patients and educate families about treatment goals and progress. Ability to work independently while collaborating seamlessly with multidisciplinary teams for holistic patient care. Experience in discharge planning, acute care environments, outpatient clinics, inpatient hospital settings, and home & community care services is highly valued. Pay:

$35.00 - $42.00 per hour Expected hours: 8.0 - 24.0 per week
