Renewal Rehab is seeking a compassionate Physical Therapist to provide evidence-based care for seniors in skilled nursing and post-acute care facilities. Responsibilities Evaluate senior patients in skilled nursing and post-acute settings using standardized assessments. Develop and implement individualized, evidence-based treatment plans to restore function and mobility. Provide gait, balance, strengthening, and functional training to reduce falls and improve independence. Document assessments, goals, and treatment progress accurately in the EMR and per regulatory standards. Collaborate with nursing, physicians, and rehab team members to coordinate comprehensive patient care. Educate patients, families, and caregivers on home exercise programs and safe mobility strategies. Participate in case conferences, care plan meetings, and quality improvement initiatives. Mentor and support therapy team members, contributing to a collaborative, patient-centered culture.
