In this flexible position, you will assess, develop, and implement personalized treatment plans to promote mobility, reduce pain, and enhance overall quality of life for residents of all levels. Your expertise will support recovery and rehabilitation efforts in our rehab programs. This role offers the opportunity to collaborate with multidisciplinary teams, utilize your clinical skills, and make a meaningful difference in residents lives whenever your expertise is needed. Duties Conduct comprehensive resident evaluations to determine functional limitations and develop tailored treatment plans aligned with clinical documentation standards. Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to improve patient mobility and reduce pain. Collaborate closely with healthcare professionals including physicians, nurses, PTA and occupational therapists to coordinate care plans and discharge planning strategies. Document all resident interactions accurately within electronic health records (EHR) systems while adhering to medical terminology standards and clinical documentation protocols. Assist in patient education by communicating treatment goals, home exercise programs, and community care options effectively. Manage care for diverse diagnosis and chronic conditions. Support outpatient clinic operations by participating in treatment plan development, patient rehabilitation sessions, and outpatient experience activities. Skills Extensive knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practices. Proven experience working with geriatric patients, pediatric populations, or neurological conditions in outpatient or inpatient settings. Strong ability to develop effective treatment plans tailored to individual patient needs across various environments such as hospitals, clinics, or home health settings. Proficiency in managing electronic health records (EHR) or electronic medical records (EMR) systems with attention to detail in documentation standards. Excellent client communication skills to foster trust and motivate patients throughout their rehabilitation journey. Ability to collaborate seamlessly with healthcare professionals for comprehensive patient care coordination including discharge planning. Familiarity with outpatient rehabilitation center procedures, inpatient hospital protocols, home & community care practices, and outpatient clinic workflows.
