Job Description

Olympia Fields Hospital seeks a licensed Physical Therapist to provide evidence-based rehabilitation services to diverse adult and geriatric patients across inpatient and outpatient settings. Responsibilities include evaluating functional limitations, developing and implementing individualized treatment plans, delivering therapeutic exercises and modalities, educating patients and families, documenting in the EMR, and collaborating with an interdisciplinary team. The role supports clinical excellence, safety, mentorship, and continuous learning in a compassionate, community-focused environment. Responsibilities Perform comprehensive PT evaluations and functional assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercise, manual therapy, and modalities Deliver gait, balance, and functional mobility training Educate patients and families on home programs and safety Document care accurately in the EMR and maintain records Collaborate with physicians, nurses, and rehab team members Contribute to quality, safety, and performance improvement initiatives Required Skills Physical therapy evaluation Treatment planning Therapeutic exercise Manual therapy Neuromuscular re-education Gait and balance training Pain management modalities EMR documentation Patient and family education Interdisciplinary collaboration
