Responsibilities Develop and implement comprehensive treatment plans tailored to individual patient needs across various settings such as outpatient clinics, hospitals, inpatient units, and home health environments Conduct thorough assessments of patients' physical conditions, including physiology and anatomy knowledge, to inform effective therapy strategies Manage clinical documentation standards by accurately recording patient progress, treatment details, and therapy outcomes within electronic health records (EHR) systems or electronic medical records (EMR) systems Provide patient education on injury prevention, mobility exercises, and self-care techniques to empower individuals in their recovery journey Assist in discharge planning by evaluating patient progress and recommending appropriate follow-up care or community resources Utilize specialized techniques such as trigger point therapy and physiotherapy modalities to optimize rehabilitation results Skills Proven experience practicing in a physical therapy role within outpatient clinics, inpatient facilities, hospitals, or home health settings Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology Skilled in developing effective treatment plans that incorporate patient goals and clinical best practices Excellent client communication skills to educate patients clearly and motivate adherence to therapy programs Familiarity with outpatient rehabilitation center operations and outpatient experience is highly desirable Proficiency in managing physical therapy documentation accurately while adhering to clinical documentation standards Join us in making a meaningful difference by restoring mobility and enhancing quality of life for our patients!
