In this vital role, you will assess, develop, and implement personalized care plans to improve patients' mobility, strength, and overall well-being. Your expertise will help patients recover from injuries, surgeries, or manage chronic conditions across various settings including inpatient hospital units, outpatient clinics, home health environments, and community care programs. This position offers an exciting opportunity to make a meaningful difference in patients' lives through compassionate and evidence-based physiotherapy interventions. Responsibilities Conduct thorough patient evaluations to determine functional limitations and develop tailored treatment plans aligned with medical goals. Implement a variety of physiotherapy techniques such as manual therapy, trigger point therapy, exercises, and modalities to promote healing and restore movement. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across care settings. Provide education to patients and their families on injury prevention, treatment procedures, and home exercise programs to support ongoing recovery. Document patient progress accurately using Electronic Medical Record (EMR) systems while maintaining detailed care plans and treatment notes. Monitor patient responses to therapy interventions and adjust treatment strategies accordingly for optimal outcomes. Assist in acute care scenarios by managing inpatient cases with a focus on rapid recovery and mobility enhancement. Deliver specialized care in pediatrics, geriatrics, orthopedics, outpatient clinics, hospital environments, home health visits, and community-based programs. Participate in team meetings to discuss patient progress, discharge planning, and ongoing care strategies. Stay current with advancements in physiotherapy practices by engaging in continuous professional development. Requirements Bachelor's or Master's degree in Physiotherapy or an equivalent qualification recognized by relevant licensing authorities. Valid licensure or registration as a licensed Physical Therapist within the applicable jurisdiction. Proven experience working in inpatient hospital settings, outpatient clinics, home health environments, or community care programs. Strong knowledge of anatomy, physiology, medical terminology, and rehabilitation techniques including trigger point therapy. Experience with EMR systems for documentation and patient management is essential. Familiarity with discharge planning processes and care coordination across multiple healthcare settings. Excellent patient care skills with the ability to communicate effectively with diverse populations including pediatrics and geriatrics. Knowledge of acute care procedures and the ability to adapt treatments for different age groups and conditions. Prior experience in orthopedics, sports injuries, or post-surgical rehabilitation is preferred.
