Rehab Strategies is expanding throughout Indiana and seeking a Physical Therapist (PT) to join our growing rehabilitation team. This position offers PRN flexibility with the potential for consistent or full-time hours , making it a great opportunity whether you're looking for supplemental income or a more substantial weekly schedule. You'll work directly with patients in a skilled nursing and rehabilitation environment, helping them regain strength, mobility, independence, and confidence following illness, injury, hospitalization, or surgery.

Responsibilities:

Evaluate patients and develop individualized physical therapy plans of care Provide skilled treatment to improve mobility, strength, balance, endurance, and functional independence Address gait, transfer, and mobility limitations Recommend and train patients on appropriate assistive devices Establish measurable functional goals and monitor patient progress Modify treatment plans based on patient response and clinical needs Complete accurate and timely clinical documentation Collaborate with physicians, nursing, OT, SLP, families, and other members of the interdisciplinary team Provide patient and caregiver education Participate in discharge planning and recommendations for continued care

Qualifications:

Graduate of an accredited Physical Therapy program Active Indiana Physical Therapist license or ability to obtain Skilled nursing, post-acute, rehab, or geriatric experience preferred Strong clinical judgment and communication skills Ability to work independently while collaborating with an interdisciplinary team New graduates are welcome to apply Why Rehab Strategies? Pay:
