Duties Conduct comprehensive patient evaluations to determine functional limitations and develop personalized treatment plans. Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics care. Manage discharge planning processes to ensure smooth transitions from hospital or inpatient settings to outpatient or home care. Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards. Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient outcomes. Provide patient education on rehabilitation exercises, injury prevention strategies, and health maintenance practices. Assist in outpatient rehabilitation programs at clinics or outpatient centers specializing in pediatrics, geriatrics, orthopedics, or neurology cases. Experience (not a requirement) Proven experience practicing as a licensed physical therapist in various settings such as outpatient clinics, hospitals, inpatient units, or home health environments. Demonstrated expertise in treatment plan development for diverse patient populations including geriatric patients, pediatric cases, and neurological conditions. Familiarity with acute care experience and inpatient hospital workflows to support complex medical cases. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques relevant to client rehabilitation. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination. Ability to communicate effectively with patients of all ages and backgrounds while providing compassionate patient care. Knowledge of discharge planning procedures and working collaboratively within multidisciplinary healthcare teams to ensure comprehensive patient support.
