Job Description
Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. In this vital role, you will assess, plan, and implement individualized treatment plans to promote recovery, improve mobility, and enhance the quality of life for patients across diverse settings. Your expertise will help patients regain independence and achieve their health goals through evidence-based physiotherapy practices. This position offers an engaging opportunity to work closely with a multidisciplinary team in outpatient clinics, inpatient hospitals, home health environments, and community care settings. Responsibilities Conduct comprehensive patient evaluations to determine physical abilities, limitations, and treatment needs. Develop personalized care plans that align with clinical standards and patient goals, incorporating techniques such as trigger point therapy and orthopedics. Implement therapeutic interventions including manual therapy, exercise programs, mobility training, and neurological rehabilitation. Collaborate effectively with healthcare professionals such as physicians, nurses, occupational therapists, and social workers to coordinate patient care. Document all patient interactions accurately within electronic health records (EHR) systems while adhering to clinical documentation standards. Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing recovery. Manage discharge planning by preparing patients for transition to home or other care facilities while ensuring continuity of care. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments. Strong knowledge of physiology, anatomy, neurology, orthopedics, pediatrics, geriatrics, and medical terminology. Demonstrated ability to develop effective treatment plans tailored to individual patient needs across diverse populations including geriatric and pediatric patients. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and care coordination. Familiarity with acute care protocols, inpatient procedures, outpatient rehabilitation practices, and community-based care models. Excellent client communication skills with a focus on patient education and engagement in their rehabilitation process. Ability to collaborate seamlessly with multidisciplinary teams to optimize patient outcomes.
