As a Physical Therapist, you'll have the opportunity to make a meaningful difference in patients' lives while working alongside a supportive interdisciplinary team dedicated to delivering exceptional care in the home setting. This position requires regular travel between Lawrence and Topeka to support patient needs. Working collaboratively with physicians and interdisciplinary team members, you will help maximize patient independence, safety, and overall quality of life. What You'll Do Complete initial patient evaluations within 48 hours of referral and conduct re-evaluations every 30 days to ensure appropriate plan of care. Develop, implement, and modify individualized treatment plans in collaboration with physicians and interdisciplinary team members. Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries. Provide education to patients and families regarding safe mobility techniques, adaptive equipment, fall prevention, and home exercise programs. Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, high-quality care. Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes. Qualifications Current Kansas Physical Therapist License required. Graduate of an accredited Physical Therapy program (APTA or AMA approved). Home Health experience and at least two years of clinical experience preferred. Current CPR certification required. Valid driver's license, current auto insurance, and reliable transportation. Demonstrated ability to work independently while maintaining strong communication with the care team.
Pay:
