Part time/PRN home health physical therapy. 1099 contract work with flexible schedule. Gonzales, Prairieville, St. Amant areas. In this vital role, you will assess individual needs, develop personalized care plans, and deliver targeted physiotherapy interventions across diverse settings. Working in home health, your expertise will foster recovery and promote independence for patients of all ages. This paid position offers an exciting opportunity to apply your physiology knowledge and clinical skills in a collaborative and supportive atmosphere. Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop tailored care plans. Implement evidence-based physiotherapy techniques, including orthopedics, geriatrics, neuromuscular rehabilitation. Provide therapeutic interventions in the home health environment to optimize patient outcomes. Document patient progress accurately using electronic medical records (EMR systems) and communicate effectively with multidisciplinary teams. Educate patients and caregivers on injury prevention, exercise routines, and self-care strategies to promote long-term health. Stay current with advancements in physiotherapy practices, medical terminology, and anatomy knowledge to deliver high-quality care. Skills Strong understanding of physiology, anatomy, and medical terminology relevant to physiotherapy practice. Proven experience in discharge planning and care coordination with other disciplines. Skilled in managing diverse patient populations including geriatrics and orthopedic cases. Familiarity with EMR systems for efficient documentation and patient tracking. Excellent patient care skills with the ability to motivate and support individuals through their recovery journeys. Experience in home & community care environments with a focus on home health services. Knowledge of orthopedic rehabilitation methods.
