Clinical Integrity:

Solid infrastructure with a small-team, \"ground floor\" feel. What you'll be doing: Conduct initial evaluations and develop individualized plans of care. Complete accurate OASIS assessments and timely documentation. Coordinate care planning with physicians and our clinical team. Supervise PTAs as needed per Massachusetts standards. What we're looking for:

Licensure:

Active Massachusetts PT license in good standing.

Experience:

1+ years in home health, outpatient, or rehab preferred.

Reliability:

Valid MA driver's license and reliable transportation.

Availability:

Able to commit to at least 2-4 visits per week (flexible timing).

Knowledge:
