In this vital role, you will develop and implement personalized treatment plans, collaborate with multidisciplinary teams, and deliver exceptional patient care across various settings. Responsibilities Assess patients' physical conditions by conducting comprehensive evaluations of mobility, strength, and functional abilities. Develop and implement individualized treatment plans tailored to each patient's needs, including exercises, manual therapy, and modalities such as trigger point therapy. Monitor patient progress regularly, adjusting care plans to optimize outcomes and ensure safety. Collaborate effectively with healthcare professionals—including physicians, nurses, occupational therapists, and social workers—to coordinate care and discharge planning. Maintain accurate clinical documentation adhering to healthcare standards for electronic health records (EHR) management and medical terminology accuracy. Educate patients and their families on treatment techniques, home exercises, and community resources to support ongoing recovery. Manage discharge planning processes by assessing patient readiness for home or community care settings and providing necessary guidance. Requirements Valid licensure or certification to practice as a Physical Therapist in the respective jurisdiction. Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology. Demonstrated ability to develop effective treatment plans for diverse patient populations including geriatric patients and those requiring outpatient rehabilitation or inpatient care. Experience with outpatient rehabilitation center environments and outpatient clinic experience is highly desirable. Proficiency in electronic health records (EHR) systems or electronic medical records (EMR) systems for documentation purposes. Excellent client communication skills with the ability to educate patients on physiotherapy techniques and care plans clearly. Familiarity with acute care experience, hospital experience, inpatient experience, outpatient experience, home & community care experience preferred. Knowledge of discharge planning procedures and working with multidisciplinary teams to ensure comprehensive patient rehabilitation.
