Physical Therapist - Home Health Flexible Schedule | Competitive Pay | Meaningful Work Pivot Point Homecare is expanding in the Framingham, MA and surrounding area and is seeking a dedicated Physical Therapist to join our growing home health team. This role offers the opportunity to deliver one-on-one, personalized care to patients where they feel most comfortable—their homes. About the
Role:
As a PT with Pivot Point, you'll support patients in regaining mobility, independence, and quality of life. You'll assess functional needs, build customized care plans, and implement therapeutic strategies that meet each individual's goals and environment.
What You'll Do:
Conduct comprehensive in-home physical therapy assessments Develop and manage individualized treatment plans Provide hands-on therapy and monitor patient progress Educate patients and caregivers on techniques and exercises Document care accurately and maintain compliance standards Collaborate closely with nurses, physicians, and support staff
What You'll Need:
MA-licensed Physical Therapist CPR certification and ability to pass a background check Excellent communication and documentation skills Reliable transportation to visit patients in the Lowell area Strong clinical judgment and ability to work independently Why Pivot Point?
