Gardner Rehabilitation and Nursing is currently seeking a compassionate and motivated Physical Therapist (PT) to join our dynamic rehab team for a special 20 week Assignment to cover a leave. We are a skilled nursing facility dedicated to providing exceptional care and rehabilitation services to our residents.

Responsibilities:

Evaluate residents and develop individualized treatment plans to improve physical function and mobility. Deliver high-quality physical therapy services in accordance with physician orders. Collaborate with interdisciplinary teams to ensure continuity of care. Document resident progress and adjust care plans as needed.

Requirements:

Valid Massachusetts Physical Therapist license (Required) Experience in a skilled nursing or long-term care setting preferred Excellent communication, documentation, and organizational skills A passion for working with elderly and rehabilitation patients
