Job Description

Hebrew SeniorLife, a Boston-based nonprofit affiliated with Harvard Medical School, seeks a per diem Physical Therapist to provide high-quality rehabilitation services in our long-term care setting. You will evaluate and treat older adults, develop individualized care plans, collaborate with an interdisciplinary team, and support residents' mobility, safety, and functional independence. Responsibilities Evaluate residents' functional status and mobility in a long-term care setting Develop and implement individualized treatment plans for older adults Deliver evidence-based therapeutic exercises and interventions Educate residents and families on safety, mobility, and home programs Collaborate with an interdisciplinary care team to optimize outcomes Accurately document evaluations, progress notes, and care plans in EHRParticipate in care conferences and contribute to discharge planning Ensure compliance with regulatory and documentation standards Recommend and train residents in use of assistive devices Support a culture of dignity, respect, and inclusion for residents and staff Required Skills Geriatric physical therapy Long-term care rehab Patient evaluation and assessment Therapeutic exercise planning Balance and fall-prevention training Assistive device training Interdisciplinary care planning Electronic health records documentation Patient and family education Regulatory and documentation compliance
