In this energetic and rewarding position, you will assess patient needs, develop personalized care plans, and implement effective therapeutic interventions across diverse clinical settings. Your expertise will help patients regain independence, improve quality of life, and achieve their rehabilitation goals through compassionate, evidence-based care. Duties Conduct comprehensive evaluations of patients' physical conditions, including mobility, strength, and functional abilities. Develop and implement tailored care plans that align with clinical best practices and patient goals. Administer various physiotherapy treatments such as manual therapy, trigger point therapy, and therapeutic exercises. Monitor patient progress regularly and adjust treatment strategies to optimize outcomes. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless patient transitions across inpatient, outpatient, home health, or community settings. Maintain accurate clinical documentation adhering to standards for medical terminology and electronic health records (EHR) management. Educate patients and caregivers on injury prevention, physiotherapy techniques, and home exercise programs to support ongoing recovery. Requirements Valid licensure or certification to practice as a Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within inpatient hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology. Experience working with geriatric patients and familiarity with outpatient rehabilitation center protocols. Ability to perform discharge planning effectively while managing care plans for diverse populations including pediatric and neurological patients. Proficiency in EMR systems or electronic health records (EHR) management for accurate documentation of patient care activities. Excellent communication skills to collaborate with healthcare teams and educate patients on treatment regimens. Prior experience in acute care or inpatient hospital environments is highly desirable.
