Home Health Physical Therapist Malden-Melrose - Saugus Territory $10,000 Sign-On Bonus Great physical therapy is about more than completing exercises—it's about helping someone safely climb their stairs, move confidently through their home and return to the routines that matter most. As a Home Health Physical Therapist with Elara Caring , you'll deliver personalized care to patients throughout the Malden-Melrose territory. You'll have the independence to manage your day, the time to focus on one patient at a time and the support of an experienced interdisciplinary team. What You'll Do Evaluate patients' mobility, strength, balance and functional abilities Develop individualized treatment plans focused on meaningful, achievable goals Provide hands-on therapy that promotes safety and independence Help patients recover following illness, injury, surgery or hospitalization Educate patients and caregivers on exercises, equipment and fall prevention Monitor patient progress and adjust treatment plans as needs change Collaborate with physicians, nurses, therapists and clinical leaders Complete accurate and timely clinical documentation Provide guidance and clinical support to therapy team members when appropriate A Career Designed to Support You $10,000 sign-on bonus to welcome you!
