Job Description

Tender Touch Rehab Services seeks a compassionate Physical Therapist to provide individualized, evidence-based care to seniors in skilled nursing and long-term care settings. In this role, you will evaluate patients, develop and implement personalized treatment plans, and focus on improving function, mobility, and independence. You will collaborate closely with an interdisciplinary team, contribute to care planning, and maintain accurate, compliant documentation. Responsibilities Evaluate senior patients and develop individualized treatment plans focused on function and independence Deliver evidence-based interventions including therapeutic exercise, gait and balance training Collaborate with an interdisciplinary team to coordinate comprehensive, patient-centered care Document evaluations, progress notes, and discharge summaries in accordance with regulatory and payer requirements Educate patients, families, and caregivers on home programs, safety, and functional strategies Participate in care plan meetings and communicate regularly with nursing and medical staff Contribute to a supportive, dignified environment that respects seniors and promotes quality of life Engage in professional development, mentorship, and quality improvement activities Required Skills Geriatric physical therapy Treatment planning and care plan development Functional mobility and gait training Therapeutic exercise and neuromuscular re-education Balance and fall prevention Wound care and pain management basics Patient and family education Interdisciplinary team collaboration Clinical documentation and EMR use Knowledge of Medicare and reimbursement guidelines
