We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team and make a meaningful impact every day. Position Summary The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions to improve mobility, strength, balance, and overall functional independence. Working closely with an interdisciplinary team, the Physical Therapist plays a vital role in helping residents achieve their rehabilitation goals. Key Responsibilities Evaluate residents' physical abilities and develop individualized treatment plans. Provide physical therapy services in accordance with physician orders and established care plans. Monitor resident progress and modify treatment plans as appropriate. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals. Educate residents, families, and caregivers on treatment goals and home exercise programs. Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. Participate in interdisciplinary care planning meetings. Ensure compliance with all state, federal, and professional practice standards. Qualifications Current Physical Therapist (PT) license in the State of MA. Graduate of an accredited Physical Therapy program. Previous experience in skilled nursing, rehabilitation, or long-term care is preferred. Strong clinical assessment and treatment planning skills. Excellent communication and interpersonal abilities. Ability to work independently while collaborating effectively with an interdisciplinary team. Passion for providing compassionate, resident-centered care.
